Dr. Jhanvi Shah is a board-certified pharmacotherapy specialist with over eight years of clinical experience in hospital, ICU, and emergency medicine settings. She currently serves as a Critical Care Pharmacist at Northwestern Palos Hospital and previously held leadership roles, including PGY-1 Residency Program Director at Loyola Medicine- --MacNeal Hospital. Her areas of expertise span pharmacy malpractice, medication error, drug overdose, adverse drug reactions, drug-drug interactions, failure to monitor, improper dosing, contraindicated medication use, and delayed administration of critical medications. Dr. Shah regularly provides expert reviews for cases involving opioid prescribing and management, anticoagulant errors (including heparin and warfarin mismanagement), insulin overdoses, failure to adjust medication for renal or hepatic impairment, and breaches in the pharmacy standard of care.  She has extensive experience and can advise on legal claims involving failure to identify allergies, incorrect medication labeling, failure to follow hospital protocols, improper compounding, and breaches in sterile technique. Dr. Shah is also frequently consulted on pharmacy negligence claims involving sepsis protocols, stroke code response (including tPA and thrombolytic therapy), antimicrobial stewardship failure, and medication errors in pediatric and geriatric populations. Her work includes analysis of medication administration records, order verification processes, and pharmacy technician supervision practices. Dr. Shah has served as an educator at Midwestern University, mentored pharmacy and medical residents, and has contributed to protocol development, quality improvement initiatives, and medication safety research presented at national conferences.  She is licensed in Illinois and maintains active ACLS and PALS certifications. Dr. Shah is available to provide expert witness services including case merit review, medical record analysis, deposition testimony, and trial testimony for both plaintiff and defense attorneys in matters involving pharmacy or medication errors, hospital pharmacy negligence, failure to meet the standard of care, and preventable adverse drug events.
